Patient Registration Information
Please arrive 15 minutes early for patient registration. Unlike most physician practices, our office generally runs very close to the scheduled appointment times. Please note: if you arrive more than 15 minutes late, your appointment may be rescheduled.
All office and diagnostic testing appointments are confirmed by phone 48 hours prior to the appointment.
Failure to cancel a scheduled office appointment within 24 hours will result in a $35.00 fee charged to your account. See the Financial Responsibility Agreement below for more details, or click here to view.

- Photo ID
- CURRENT Insurance card(s)
- Completed patient forms
- Your co-payment or co-insurance for a specialist (we accept cash, check or credit card)
- List of all of your medicines
